REMEMBER: Personal consultations should NEVER take the place of your usual health care provider!

INDIVIDUAL CONSULTATIONS

Women seeking individual consultations can make an appointment in the New York/New Jersey area by calling 212-724-3005. A one and a half hour initial consultation is $375.00. Follow up 30 minute consultations are $150.00. These fees seem high – even to me – but unfortunately it reflects the cost of doing business in this part of the country. TELEPHONE CONSULTATIONS

A more reasonable option is a telephone consultation. If interested, email me using the following address: camillo@innerestrogen.net.  Include your full name, address, telephone number and reason for the consultation. An initial 1 ½ hour telephone consultation is $195.00. Follow up 30 minute consultations are $95.00. These are NEVER reimbursed by insurance companies.

INSURANCE INFORMATION

I do not accept any insurance however you will receive a receipt for services that can be submitted to your insurance. Although many insurance companies will reimburse a portion of this amount, there is no guarantee. Always check with your insurance company first if this is a significant concern. Payment is requested with mailed completion of an intake form prior to the visit or telephone consultation. You will receive a receipt which can be used as a medical deduction for tax purposes.

IMPORTANT: ALL CONSULTATIONS REQUIRE THAT YOU HAVE AN IDENTIFIED PRIMARY CARE PROVIDER. In order to participate in this service, you must have a referral from your health care provider. I will send a summary of the consultation to this person so that you receive the appropriate follow up and continued care.
